Notice to Texas Consumers

Thank you for choosing to participate in Citizen Solutions™.

This is our Notice to Texas Consumers regarding the privacy of the identifiable health information participants in the Citizen Solutions program may voluntarily choose to share with Builders Network, Inc. (“Builders”).

This Notice applies to residents of Texas only.

Builders is not a “covered entity” under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and is not otherwise subject to HIPAA. Builders is not a health care provider and does not maintain medical records. Builders maintains, and this Notice applies to, only health care stories and information that participants choose to voluntarily share with us, and which are identifiable to you (referred to as “Health Information”).

You are not required to share identifiable health care information with us in order to participate in Citizens Solutions. The Health Information you share with us is at your sole discretion.

1. Notice of Electronic Disclosure of Health Information. The Health Information you share with Builders may be electronically disclosed by Builders as described in the Terms of Participation & Release and as otherwise permitted or required by law. For example, the Health Information may be shared with certain service providers we utilize to operate the Citizen Solutions program.

2. How to Request Your Health Information While Builders does not have access to or retain your medical records, you may request access to the Health Information you provide to us. To request your Health Information from us, please follow these instructions:

  • Send your request to: ima@buildersmovement.org 
  • Include: your full name, contact information, and the preferred format for receiving them (we will provide the Health Information you request in the format you prefer if feasible).

Note that because you alone choose what Health Information you disclose to us, not all Health Information will be readily linkable to name or contact information. Builders may require additional information from you to identify your Health Information and fulfill your request.

3. How to File a Complaint. If you have a concern with our privacy practices, you may file a complaint with the Texas Attorney General.
